Admission Guidelines

Declaration of Admission Policy

St. Alexius College - High School Program is open to learners who are capable of undergoing High School training and who are willing to abide by the rules and regulations of the institution. The High School Program reserves the right to refuse admission to any learner whose qualifications do not meet the standards and requirements of the program in which the learner desires to enroll.

General Requirements

  • Original High School Report Card (Form 138)
  • Original Student’s Permanent Record (Form 137)
  • Certificate of Junior High School/Grade School Completion
  • Certificate of Academic Honors from the Principal/Guidance Office
  • Certificate of Good Moral Character (Original)
  • If applicable: Photocopy of the result of the evaluation (Developmental Pediatrician/Child Psychologists, Reading Specialists and other support services result/s as required by SAC ISD)
  • 2 pieces of 2X2 picture (white background)
  • long brown envelopes
  • National

  • National Career Assessment Examination (NCAE – photocopy)
  • Educational Service Contracting (ESC) Certificate
  • Qualified Voucher Recipient (QVR) Certificate for Non-ESC Grantees
  • Transferee Learners are those who wish to transfer & enroll in the department after having been enrolled in other institutions.

  • Alien Certificate of Registration (photocopy)
  • Photocopy of Student’s Passport showing the date and place of birth
  • Photocopy of Student’s Visa (authenticated by the Philippine Embassy in the country of origin)
  • Shifters are learners who have secured permits to change tracks or strands should follow the following procedures:

  • Get Shifting Form and Evaluation Sheets from the Registrar
  • Accomplish the Shifting Form and Evaluation Sheets
  • Process change of track or strand at the Registrar’s Office
  • Receive the copy of the fully processed Shifting Form and forward it to the accepting Adviser
  • The school must be notified at least 30 days before a Learner stops or transfers to another school.
  • The Learner must complete the official Withdrawal Form and by submitting the same document to the Registrar.
  • If a Learner drops out or withdraws enrolment:
  • Official notification from parents should be submitted to the Business Office;
  • The Registrar will provide the Learner with a withdrawal form;
  • The Learner will complete the procedures in the withdrawal form; and
  • The Learner must submit the fully accomplished withdrawal form to the Registrar’s Office for processing of final exit documents.
  • The submission date reflected in the withdrawal form received by the Registrar serves as the date that the Learner officially dropped out.
  • In case of failure to process Enrolment Withdrawal, regular charges will be applied.
  • If a Learner officially withdraws within the first week after classes have started:
  • He/she can process his/her payment in the Business Office for a refund; and
  • He/she will receive a 75% reimbursement regardless of his/ her attendance.
  • If a Learner withdraws within two weeks after classes have started, he/she cannot process his/her payment in the Business Office for a refund. 50% of his/her payment will be reimbursed regardless of his/ her attendance.
  • If a Learner withdraws within the third week after classes have started, he/she cannot process any more for a refund of his/her payment to the Business Office. The Learner will be charged all the school fees for the entire school year.
  • Learners who desire to transfer to another school in the middle of the academic year, shall forfeit his/her subsidy from the government.
  • Learners who desire to transfer to another school at the end of the school year must apply for Certificate of Transfer Credentials which shall be issued only after the applicant has been cleared of all financial, library, and property liabilities.
  • Learner’s ID must be surrendered to the Principal’s Office.
  • The Office of the Registrar issues an Exit Clearance Slip to the applicant.
  • After the release of the Certificate of Transfer Credentials, the Learner shall no longer be connected with the school.

  • Physical Exam/Laboratory Exams/Chest X-Ray/Drug Test from DAPPMC (for allied health courses only)
  • Drug Test
  • Those who stopped schooling for reasons other than disciplinary and academic problems.

  • Updated Transcript of Records and Evaluation sheet
  • Physical Exam, Laboratory Exams, Chest X-ray, Drug Test from DAPPMC
  • Drug Test
  • Evaluation sheet of the previous semestral performance
  • ID card for validation purposes
  • Physical Exam, Chest X-ray results, Drug Test from DAPPMC
  • Immunization record for allied health courses
  • Pregnancy Test for practicum/interns (as applicable)
  • Drug Test
  • Cross-Enrolled Students from other colleges or universities will be admitted upon presentation of a Permit to Study signed by the registrar of the school last attended. This cross-enrollment does not preclude the right to transfer to the College.

    Shifters are students who have permit to change course should follow the following procedures:

  • Secure Shifting Form and evaluation sheets from the recommending Dean/Program Head and submit these documents to the accepting Dean/Program Head.
  • Process change of status at the Registrar’s Office.
  • Proceed to enrollment.
  • Foreign students from recognized institutions overseas may be admitted to the college upon presentation of admission credentials.

    The students must submit the following requirements for application to conversion of student visa:

  • Photocopy of the student’s passport showing biopage (date and place of birth), admission page and duly authorized stay of at least twenty days;
  • Original copy of Medical Certificate issued by the Bureau of Quarantine to certify that the applicant is not afflicted with any contagious disease and is mentally fit;
  • National Intelligence Coordinating Agency (NICA) clearance;
  • Bureau of Immigration (BI) Clearance Certificate;
  • Duly notarized Letter of Intent from the Applicant;
  • Duly notarized General Applicant Form accomplished by the applicant (BI Form No MCL-07-01);
  • Original copy of the Notice of Acceptance (NOA) containing a clear impression of the school’s official dry seal or a duly notarized written endorsement from the school for the conversion of the applicant’s status signed by the school’s Registrar;
  • Transcript of Records/Scholastic Records duly authenticated by the Philippine Foreign Service Post (PFSP) located in the student-applicant’s country of origin or legal residence.
  • A notarized Affidavit of Support including bank statements or notarized grant for institutional scholars, to cover expenses for the student’s accommodation and subsistence, as well as school dues and other incidental expenses.
