• Get 𝗘𝗻𝘁𝗿𝗮𝗻𝗰𝗲 𝗘𝘅𝗮𝗺𝗶𝗻𝗮𝘁𝗶𝗼𝗻 𝗙𝗼𝗿𝗺 from the Guidance Office
• Pay Php 150.00 at the Cashier
• Proceed to Guidance Office Testing room
• Show the filled-up form together with the OR to the proctor
• Make sure to bring a working 𝗕𝗔𝗟𝗟𝗣𝗘𝗡 𝗮𝗻𝗱 𝗣𝗘𝗡𝗖𝗜𝗟
