1. Incoming Freshmen

Requirements:

  • Entrance examination (OLSAT)
  • Interview
  • High School Card (Form 138)
  • PSA – Authenticated Birth Certificate (Photocopy)
  • Certificate of Good Moral Character
  • Physical Exam/Laboratory Exams/Chest X-Ray/Drug Test
  • from DAPPMC (for allied health courses only)
  • Drug Test from DAPPMC (for non-health related courses only)
2. Transferees
Transfer students are those who wish to transfer & enroll in the college after having been enrolled in other institution.
  • Interview with the College Dean/Program Heads
  • Entrance examination (OLSAT)
  • Transfer Credentials
  • Updated transcript of records for evaluation (Certification Authentication Verification)
  •  PSA-Authenticated Birth Certificate (Photocopy)
  • Certificate of Good Moral Character
  • Physical Exam, Laboratory Exams, Chest X-Ray, Drug Test from DAPPMC (for health related courses only)
    from DAPPMC (for health related courses only)
  • Drug Test from DAPPMC (for non-health related courses only)
3. Returnees are those who stopped schooling for reasons other than disciplinary and academic problems.
  • Updated Transcript of Records and Evaluation sheet
  • Physical Exam, Laboratory Exams, Chest X-ray, Drug Test from DAPPMC
  • Drug Test from DAPPMC (for non-health related courses only)
4. Continuing Students
  • Evaluation sheet of the previous semestral performance
  • ID card for validation purposes
  • Physical Exam, Chest X-ray results, Drug Test from DAPPMC
  • Immunization record for allied health courses
  • Pregnancy Test for practicum/interns (as applicable)
5. Cross-Enrolled Students from other colleges or universities will be admitted upon presentation of a permit to study signed by the registrar of the school last attended. This cross-enrollment does not preclude the right to transfer to the College.
6. Shifters are students who have permit to change course should follow the following procedures:
  • Secure Shifting Form and evaluation sheets from the recommending Dean/Program Head and submit these
    documents to the accepting Dean/Program Head.
  • Process change of status at the Registrar’s Office.
  • Proceed to enrollment.
7. Foreign Students from recognized institutions overseas may be admitted to the college upon presentation of admission credentials. The Students must submit the following requirements:
  • Photocopy of the student’s passport showing date and place of birth and birth certificate or its equivalent duly authenticated by the PFSP.
  • Transcript of Records/Scholastic Records duly authenticated by the Philippine Foreign Service Post (PFSP) located in the student-applicant’s country of origin or legal residence.
  •  A notarized Affidavit of Support including bank statements or notarized grant for institutional scholars, to cover expenses for the student’s accommodation and subsistence, as well as school dues and other incidental expenses.
